On 4 May 2026, the White House released the 2026 National Drug Control Strategy, the first US drug policy framework to formally commit to deploying wastewater surveillance at a national scale. The document states that the administration will prioritise "the establishment of new data systems to monitor drug consumption in real-time, through a national wastewater-based monitoring system and biosurveillance." The reasoning is laid out plainly: self-reported survey data gives an incomplete picture because people may not know what they have consumed given the widespread adulteration of the drug supply, and because of reluctance to disclose illegal behaviour. The strategy concludes that "this survey data needs to be complemented with more objective measures of drug use, such as wastewater testing and biologic specimen testing."
Part of a larger data architecture
The strategy does not position wastewater as a standalone instrument. Results from monitoring are to be combined with law enforcement seizure data, hospital and emergency department trends, toxicology reports, and mortality figures, with the Office of National Drug Control Policy (ONDCP) coordinating a new Drug Data Interagency Working Group across all relevant federal agencies. There is also an explicit role for artificial intelligence: the document describes using natural language processing to identify emerging substances in social media, which can then be "geographically confirmed by wastewater-based drug monitoring" — framing wastewater data as ground-truth validation for digital intelligence tools. The strategy names wastewater testing specifically under its data expansion priorities as a method that "identifies new and emerging illicit substances and metabolites at the community level."
WBE: a field with deep roots, now gaining policy traction
The US commitment lands in a field that is already well established. In Europe, the SCORE network has been conducting annual wastewater monitoring campaigns since 2011, with data feeding into the European Monitoring Centre for Drugs and Drug Addiction (EMCDDA). By 2023, the programme covered a record 104 cities across 21 countries, and the 2024 edition extended the analysis to 88 cities, documenting a continued rise in cocaine detections. The COVID-19 pandemic gave wastewater-based epidemiology (WBE) a global push, demonstrating its value as an early warning tool at population scale — from the early science behind SARS-CoV-2 detection in sewage to its expansion into polio and monkeypox surveillance. The U.S. built its own Wastewater Monitoring Program through the CDC during the pandemic, reaching over 1280 reporting sites. The 2026 strategy, in other words, is not starting from zero.
What the strategy does not yet say
The document is a policy commitment, not an operational plan, and should be read as such. No budget is allocated for the wastewater monitoring programme in the publicly available text. No lead agency is named for its implementation, and no timeline is specified. What the strategy does signal, clearly, is direction: a federal administration has placed WBE explicitly within its core public health toolkit, framed as an objective complement to data sources that have long been considered insufficient on their own. For utilities, laboratories, technology providers, and regulators working in this space, the question now is how implementation takes shape — and whether a formalised US national system, if it materialises, accelerates similar moves in countries still operating fragmented or pilot-scale programmes.
